Free and confidential consultation available in the home, office, or public meeting location. Consultations include:- Develop a "contingency plan" in the event of a crisis situation- Discuss individual's intentions for living arrangements as the need for services increases- Discuss in-home and community-based service options- Discuss valuable support available for caregivers as needed- Establish goals for remaining involved in social activities outside the home- Establish a plan to secure additional services as needed- Identify current and potential future support needs
