Offers a discount program for eligible patients and a payment plan option for any balance not covered by the discount. The program cannot be used for dental and pharmacy bills and charges. Program assistance is based on: - Assets, such as property or savings accounts - Family size - Income - Liabilities, such as bills or regular payments due Individuals may apply even if they have insurance coverage.

Application process

- Call; email completed applications to [email protected]; mail applications to 21112P PO Box 1309 Minneapolis, MN 55440 - Visit website for application/registration form. After the application is received, a letter is sent to the applicant within 10-14 days by mail notifying them if they qualify for the program or not. If they qualify, the amount the bill is discounted is included in the letter. The program qualification lasts three months from the approval date. Individuals need to apply again after three months to see if they qualify for the program.

Application/registration form; copies of federal income tax return; last 60 days of paystubs; financial liquid asset holdings; Social Security benefit letter; Unemployment Insurance benefit letter

- HealthPartners Clinics patients having trouble paying their HealthPartners Clinics medical bills - Individuals must first apply for and use all government program assistance - If an individual was denied government assistance based on income guidelines, they may still qualify for the HealthPartners Clinics Financial Assistance Program
