Medical Assistance

Provides assistance in connecting individuals with a qualified mental health professional to complete a diagnostic assessment and gather the documentation needed to determine eligibility for services. Case managers help individuals access medical, social, educational, vocational, and other supports based on their specific needs. The Community Support Services Program helps individuals remain safely in the community by offering supportive services such as medication monitoring, independent living skills assistance, and crisis support.
