African American persons living with or at risk of HIV/AIDS who are low income (at or below 300% of federal poverty levels); primarily serves residents of Anoka, Carver, Dakota, Hennepin, Ramsey, Scott and Washington counties

Program Fees

Clients are assigned a case manager who provides the following services:
  • Creates a care coordination plan, sets goals and assists with removals of barriers to care
  • Links clients to health care, mental health care services and other services for timely and coordinated access to medically appropriate levels of health and support services. Clients participating in MCM are also eligible for the 340B Pharmacy Program, and transportation.
