Description

MNChoices is a web-based application that is comprehensive and integrates assessment and support planning for people who need long-term services and supports. The assessment takes a person-centered approach to ensure services meet each individual's strengths, goals, preferences, and assessed needs. The program includes assessing what level of service is needed for care such as homemaker, home health aide, adult day care. Services include: - Early intervention visits - Information and education about local long term care options - Information about public programs that can help pay for programs - Pre-admission screenings prior to nursing home admission - Transition assistance for people who want to return to community settings after admission to a facility NOTE: Community or in-person screening is required in some situations to determine eligibility for waiver services.
