Provides managed care programs offering medical, behavioral health and social services including: Prepaid Medical Assistance Program (PMAP): A managed care program that offers medical, behavioral health and social services for residents of Hennepin County, between the ages of 0 and 64 and eligible for Medical Assistance (Medicaid) or MinnesotaCare. MNCare: A managed care program that covers health care for people who do not have access to affordable health care coverage and offers medical, behavioral health and social services for residents of Hennepin County, between the ages of 0 and 64 and eligible for Medical Assistance (Medicaid) or MinnesotaCare. Special Needs Basic Care (SNBC): A managed care program for individuals with disabilities who are residents of Hennepin County, between the ages of 18 and 64, eligible for Medical Assistance (Medicaid) and certified disabled (by a State Medical Review Team or through Social Security Disability Insurance).
