Provides professional services, support, and individualized health care in the home setting. Services available include: Home Health Aide:
  • Daily living activity assistance with ambulation, bathing, dressing, eating, grooming, toileting, and transferring
  • Exercises and range of motion
Homemaker:
  • Services include dusting, laundry, light housework, meal preparation, and shopping
Hospice Care:
  • Enables patients to continue an alert, pain-free life, and to manage other symptoms so that the patient's last days can be spent with dignity
  • Hospice care can be given in the patient's home, a hospital, nursing home, or private hospice facility
  • Nurse on-call 24 hours/7 days a week
Skilled Care:
  • Medical social services
  • Nursing services include comprehensive terminal care, infusion therapy, medication supervision, newborn blanket therapy, teaching, lab tests, and wound care
  • Rehabilitative therapy services such as occupational, physical, and speech therapies
