Adults age 60 and older who are residents of Duluth, Hermantown, and Proctor who: - Are able to walk alone or with the aid of a walker - Are not on Medical Assistance - Are unable to drive themselves, do not have caregivers who are able to transport them, and do not qualify for other transportation programs
8 am - 4:30 pm Monday - Thursday 8 am - 12 noon Friday
Call at least 5 - 7 business days before a scheduled appointment
