Description

Provides the following behavioral health services: - Adult Rehabilitative Mental Health Services (ARMHS) - Alpha-Stim therapy - Coordinated Entry VI-SPDAT assessments - Eye Movement Desensitization and Reprocessing (EMDR) - Hearth Connection Homeless Program/Adult Mental Health Targeted Case Management (AMH-TCM): For people who have long histories of homelessness that are interested in receiving and engaging in supportive services to help end their homeless situation. Head of household ages 18 and older must have been homeless continuously for a year or more or have experienced 4 episodes of homelessness in the past 3 years. Participants work with a case manager to find and keep affordable housing and meet other personal goals addressing areas that have contributed to homelessness. The caseworker acts as a referral source, advocate and support person for the participant, helping connect them with resources and programs. Housing subsidy vouchers are awarded based on regional availability. Applicants must complete a VI-SPDAT assessment through Coordinated Entry first before accessing this program. - Outpatient therapy for individuals and families - Play therapy -Rule 25 assessments - Special mental health services referrals - Substance use counseling and referrals
