Description

Provides a program called the Project for Assistance in Transition from Homelessness (PATH) to connect individuals with housing services and other resources to find safety and housing stability. Services provided through the program: - Helps individuals apply for economic assistance, medical assistance, and Social Security as needed - Helps individuals obtain necessary paperwork to secure housing and create an individualized housing plan - Provides referrals to medical and mental health care, legal assistance, and other essential services - Works through barriers preventing the individual from finding and keeping housing - Works with clients to access Coordinated Entry services to determine if they qualify for Project Home or Youngdahl Living programs. Project Home and Youngdahl programs are permanent support housing programs for those dealing with homelessness, unstable living conditions, and need stable housing.
