Emergency Services - Prairie Five Community Action

Emergency Shelter Program: Offers limited motel stays as a short-term solution while working with people to come up with a plan for long-term housing stability. Part of the plan may include employment and housing searches, connections to mainstream resources, and short to medium-term rental assistance. Transitional Housing Program: Provides a long-term housing program to help an individual regain self-sufficiency. The goal of this program is to transition struggling individuals and families into stabilized living situations. The resources are offered for people of all backgrounds, including single parents, children, and individuals that were recently evicted. The program requires an assessment, application approval, weekly case management meetings, and constant contact with a case manager. If approved Prairie Five can: - Pay a portion of rent ?- Provide assistance with referrals, phone calls, setting up appointments, application assistance, resume building, job search, family development, etc. ?- Tenant education ?- Budgeting/smart money use Family Homeless Prevention Assistance Program: Helps families or individuals get out of homelessness or prevent homelessness from occurring. If eligible, the FHPAP program can help with first month's rent, security deposit, or back rent/mortgage assistance up to a certain dollar amount. To be eligible for this program individuals must: - Fill out an application ?- Be able to prove a housing crisis (eviction notice, mortgage papers, etc.) ?- Must have income During this program, all applicants who are deemed eligible will also be required to watch the "Rent Wise, Money Smart" videos. The videos go over smart shopping, facts about debt and credit reports, saving, budgeting, and more.
