Eligibility

Adults must: - Have a serious and persistent mental illness - Have a significant functional impairment, frequent utilization of Emergency Departments, or hospitalizations - Have a health plan such as Medical Assistance that covers ACT services - Live in either Dakota or Ramsey County

Application process

Call during business hours for information. A provider referral is needed. Individuals should call the 0067 number if they do not have a case manager, care manager, or social worker.
