Eligibility
Residents of Hennepin County who are:- Fathers or alleged fathers of a minor child- Parents of a minor child and if one parent does not live with the child- Parents who pay court-ordered child support- Court-ordered and have physical custody of a minor child- Receiving public assistance for a minor child who lives in their home

Print Application:- Call the Print Application Form Request line for an application. Fees
$35 federal annual fee may be applicable; cost recovery fee of 2% of income; a monthly $15 is paid by the parent paying support, deducted through income withholding; vary by service and circumstances
