Eligibility

Families and individuals who are a Hennepin County resident needing assistance with or an assessment for county services, or case management. If needs are identified for ongoing services, specific program eligibility criteria will apply.

Required documents

Application/registration form birth certificate for families with children photo ID proof of residency (piece of mail, application of new ID, rental lease, bus ticket, etc.) shelter verification verification of the last 30 days of income and assets including more supporting documents can help process applications quickly

Front Door consolidates intake for most programs and connects clients to the level of service that best meets their needs. Front Door Screener's connect clients to:

- Community resources or other providers

- Education or consultation of county programs

- Intake assessments of county-operated programs. For clients identified as needing county intervention, a Front Door Service Planner will conduct a thorough evaluation of needs and provide short-term case management as well as assess client eligibility for possible ongoing services from the county or a contracted provider.
