Front Door - Hennepin County Human Services - Social Services

Front Door consolidates intake for most programs and connects clients to the level of service that best meets their needs. Front Door Screener's connect clients to: - Community resources or other providers - Coordinated Entry System (CES) assessments for families based on eligibility criteria. Families can visit the website to fill out a Families Coordinated Entry referral form. - Education or consultation of county programs - Intake assessments of county-operated programs. For clients identified as needing county intervention, a Front Door Service Planner will conduct a thorough evaluation of needs and provide short-term case management as well as assess client eligibility for possible ongoing services from the county or a contracted provider.

- Call; after-hours messages will be returned on the next business day; email responses are made within one working day - Coordinated Entry Assessment: Families in need of a CES assessment can call Front Door or fill out a Families Coordinated Entry referral form to set up an appointment - Walk into A-1400 at the Hennepin County Government Center

Application/registration form; birth certificate for families with children; photo ID; proof of residency (piece of mail, application of new ID, rental lease, bus ticket, etc.); shelter verification; verification of the last 30 days of income and assets; including more supporting documents can help process applications quickly

Eligibility

Families and individuals who are a Hennepin County resident needing assistance with or an assessment for county services, or case management. If needs are identified for ongoing services, specific program eligibility criteria will apply. Coordinated Entry System (CES) Assessments can be provided to families who: - Are in a place not meant for human habitation (living outside, on the train, car, etc.) - Are housed in a family shelter such as People Serving People or St. Anne's and will be identified by shelter staff for an assessment - Have custody of children 51% of the time or are currently pregnant - Have a County of Financial Responsibility from Hennepin County
