Offers a discount program for eligible patients and a payment plan option for any balance not covered by the discount. The program cannot be used for dental and pharmacy bills and charges. Program assistance is based on: - Assets, such as property or savings accounts - Family size - Income - Liabilities, such as bills or regular payments due Individuals may apply even if they have insurance coverage.

Application process

Visit website for application/registration form. After the application is received, a letter is sent to the applicant within 10-14 days by mail notifying them if they qualify for the program or not. If they qualify, the amount the bill is discounted is included in the letter. The program qualification lasts three months from the approval date. Individuals need to apply again after three months to see if they qualify for the program.

Application/registration form; last federal tax returns; liquid asset holdings; proof of income pay stubs within the last 60 days; Social Security income statements, letter, or unemployment benefit letter

Patients of HealthPartners Clinics; individuals must first apply for and use all government program assistance and if denied, they may still qualify for the Health Partners Clinics Financial Assistance Program
