Fee

- Vary by service - Offers a sliding fee scale to patients without insurance who qualify. Proof of family income is needed to be eligible for the sliding fee scale. Call ahead to discuss payment procedure policy. Staff will help patients apply for Medical Assistance and physician's assistance medication programs. - Donations are accepted
