Medical Expense Grant Program - United Healthcare Children's Foundation

Provides timely financial support through grants to families with children to help with medical expenses not covered, or not fully covered by a family's commercial health insurance. The grant will only cover costs as outlined by the grant approval letter once a family has been approved for funding. The amount awarded to an individual within a 12-month period is limited to $5,000. Awards to any one individual are limited to a lifetime maximum of $10,000. Grant recipients who are awarded less than $5,000 may re-apply for another grant once the current grant funds have been completely exhausted. Applications not approved by the Board must wait twelve months before re-applying unless the medical condition and requested items have significantly changed from the original request. NOTE: Families who are primarily covered under a federally funded or state funded insurance plan, such as Medicaid, that are offered to families with low-income are not eligible.

- Visit website to apply online. More information is listed on the website about exclusion areas not eligible for funding. A physician's Certification of Medical Condition form is listed on the website at www.uhccf.org/wp-content/uploads/2023/03/Physician-Form-5.17.22.pdf. - All required documents must be attached to the application prior to submitting online. The Foundation Board will review applications and meets once a month. Applications will only be reviewed by the Board from the region the application is submitted. Award notifications will be sent via email to the primary parent/guardian's email address.

Eligibility

Open to all children who meet these qualifications: - Ages birth - 16 years - Household income does not exceed maximum income as documented on IRS Tax Form 1040 - Have a Social Security number - Have a commercial health plan either through an employer or individually purchased plan - The child's care, medical services, treatments, and/or therapies must be administered by a licensed professional in the United States
