Winona County Health and Human Services offers Long Term Care Consultation to assist families in learning about resources in the community and to assist with the development of a plan of care with Public Health Nurses or Social Workers in order for individuals to remain at home when appropriate. This office also provides the mandated screening prior to nursing home placement for Minnesota residents.
