Advocates for patients and families by linking them to services and resources: - Advance Care Planning: For people to have accurate information in order to make informed decisions about treatments they would or would not want for end of life care - Domestic Abuse/Child Protection: Answers questions and takes reports of suspected cases of child abuse and domestic violence - Mental Health/Chemical Dependency: Completes a chemical dependency assessment to direct patients to appropriate treatment programs. Patients struggling with mental health issues can be referred to local counseling services or inpatient mental health programs. - Nursing Centers/Assisted Living Residences: Offers education and guidance in the selection of a nursing home or assisted living facility to meet health care needs after a hospital stay - Patient Care Conference: Patients and their families can schedule a patient care conference with a nurse, doctor or other health care provider to talk more specifically about a health care directive, treatment preferences, discharge planning, coping methods or any other concerns that arise in facing a serious illness and hospital stay - Pregnancy and infant loss resources
