Provides family home visiting services by public health nurses and family health staff to support pregnant individuals, parents, caregivers, infants, and children.- Home visits for pregnant individuals to support healthy pregnancy behaviors and preparation for birth and postpartum care- Home visits for families with newborns and young children, including education on child development, nutrition, parenting, and safety (including car seat education), and connection to community resources- Home visiting support for children with disabilities or special health care needs, including education, coordination, and referrals?- Supportive home visiting services for families experiencing parenting stress or crisis, provided or overseen by public health nursing staff; may accept referrals from families and community partnersVisit frequency is based on family need and the level of support required.
