Eligibility
  • Single individuals ages 16 and older in Hennepin County
  • Must have strong ties to Hennepin County or experiencing homelessness in Hennepin County
  • Are sleeping outside or in a place not meant for human habitation
  • Household income does not exceed 30% of the Area Median Income (AMI) for the household's size
  • Single adults staying at a homeless shelter in Hennepin County for over 14 days will be identified by shelter staff for an assessment

Provides coordinated entry assessments for single adults who are experiencing HUD homelessness (i.e., sleeping outside or in a place not meant for habitation) in Hennepin County.
