Eligibility
- Single individuals ages 16 and older in Hennepin County
- Must have strong ties to Hennepin County or experiencing homelessness in Hennepin County
- Are sleeping outside or in a place not meant for human habitation
- Household income does not exceed 30% of the Area Median Income (AMI) for the household's size
- Single adults staying at a homeless shelter in Hennepin County for over 14 days will be identified by shelter staff for an assessment

- An appointment will be scheduled to complete the assessment. Assessors can meet clients where they are located.
