- Be eligible for General Assistance
- Have long-term disability that significantly impairs employment
- Qualify as long-term homeless

Application process
- Call or walk-in to a drop-in center or shelter to connect with a street outreach worker for a Coordinated Entry Assessment
- A referral is required through the Hennepin County Coordinated Entry process. This process is open to individuals who are staying in an emergency shelter or working with a street outreach team and sleeping outdoors (camp, car, transit, etc). Individuals currently staying with friends or family, or who are in treatment, will not be able to access Coordinated Entry.

Offers supportive housing for youth who are experiencing homelessness and have a mental health and/or chemical dependency diagnosis or a physical disability.
Provides case management services including assistance with education and employment, connections to mental health and chemical dependency treatment, connections to health care services, leadership development opportunities, life skills, support groups, and youth development activities.
