  • Adults 18 and older, who have a serious and persistent mental illness; serve residents of Hennepin County
  • Vail Connect services are limited to people connected to North Memorial Health and affiliated clinics

Required documents

Community Support Programs require a completed application and verification of a serious and persistent mental illness by a mental health professional. When contacting Vail please provide: name, date of birth, physical health diagnoses, mental health diagnoses, medical records number, and reason for referral.

Care navigators help individuals, their families, and other care providers obtain mental and/or physical health assessments, treatment, support, resources, and education. It is a mobile service intended to be short-term, typically up to three months. Care navigators meet individuals in their home or elsewhere in the community to arrange services and supports.
