Coordinated Entry: An access point providing coordinated entry assessments to single adults and families with children who are low-income and meet eligibility requirements in the county area Family Homeless Prevention Assistance Program: This program helps families or individuals get out of homelessness or prevent homelessness from occurring. If eligible, the FHPAP program can help with the first month's rent, security deposit, or back rent/mortgage assistance up to a certain dollar amount. To be eligible for this program, individuals must: - Fill out an application - Be able to prove a housing crisis (eviction notice, mortgage papers, etc.) - Must have income During this program, all applicants who are deemed eligible will also be required to watch the "Rent Wise, Money Smart" videos. The videos go over smart shopping, facts about debt and credit reports, saving, budgeting, and more. Transitional Housing Program: Provides a long-term housing program to help an individual regain self-sufficiency. The goal of this program is to transition struggling individuals and families into stabilized living situations. The resources are offered for people of all backgrounds, including single parents, children, and individuals that were recently evicted. The program requires an assessment, application approval, weekly case management meetings, and constant contact with a case manager. If approved, Prairie Five can: - Pay a portion of rent - Assist with referrals, phone calls, setting up appointments, application assistance, resume building, job search, family development, etc. - Tenant education - Budgeting/smart money use
