Offers home care services to individuals with a disability or in need of medical support and assistance at home. Services include: Home health aides/homemaker services - Exercises - Laundry - Light homemaking duties - Meal planning and preparation - Nonsterile dressing changes - Personal care Home health care nurse services - Case management (evaluation and assessment of care plan) - Catheter care, dressing changes, injections - Home safety evaluation - Medication management - Ostomy care and education - Therapeutic activities Therapy - Physical therapy
