Eligibility

Adults ages 18 and older living with a mental health diagnosis and mild cognitive disability, have a functional impairment in three or more life domains due to the symptoms of a mental health diagnosis, currently receiving Medical Assistance (MA) or PMAP, and are residents of Dakota, Hennepin, and Ramsey counties

Works to promote independent living and reduce the risk of relapse and mental health hospitalization. An ARMHS case worker teaches skills to: - Access mental health services and community resources - Cope with stress, fear and anxiety - Develop a healthy lifestyle - Find safe, affordable housing - Improve interpersonal skills and communication - Learn household management skills and budgeting skills - Learn to use community resources
