Eligibility

- Older adults and adults and adolescents with developmental, emotional, or physical disabilities - Serves Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington counties - Must be enrolled in one of Minnesota's health care programs, Medical Assistance, or waiver program. Some people enrolled in Minnesota Care are also eligible. - Must have had a previous assessment by the county, tribal nation, or managed care organization that confirms eligibility

Provides employment exploration, development, and support services. - Employment exploration services: Helps individuals gain a better understanding of competitive, integrated employment opportunities and provides activities and experiences which help to strengthen their knowledge, interests, and preferences to make informed decisions about competitive employment. - Employment development services: Helps individuals find competitive, integrated employment, become self-employed or establish a micro-enterprise business. - Employment support services: Provides supports that help individuals maintain paid employment in community businesses/settings.
