Description
Provides in home support and group-based therapeutic recreation for individuals with special needs ages two through adulthood, emphasizing community involvement, friendship-building, and healthy and independent living. Programs include the following: - Healthy Families and Parents as Teachers: Provides early, relationship-based home-visiting support for families from pregnancy through age 5. Staff meet regularly with caregivers to strengthen parent-child relationships, support healthy development, and build confidence during early childhood. Visits are tailored to each family and focus on creating a foundation for learning, growth, and well-being. Services are offered weekly for up to 60 minutes, typically over 2 to 4 years. The program serves Hennepin County residents and operates Monday through Friday with flexible scheduling in community settings and from the Nicollet Center. - PASS Program Promoting Attendance and School Success: Offered in partnership with Scott County, supports students in kindergarten through fifth grade who are experiencing or are at risk of attendance concerns. Staff work directly with children, caregivers, and school personnel to understand barriers to consistent attendance and develop individualized solutions. The program focuses on collaboration, problem-solving, and helping each child be present and ready to learn. Visits typically occur twice per month in the family's home, with flexible weekday hours. Eligibility includes Scott County residents with a child in kindergarten through fifth grade facing attendance issues. - Sensitive Parenting Support: Offers in home support for pregnant individuals and caregivers of children from birth to age four who are living with a mental health diagnosis. Staff provide emotional support, developmental guidance, resource connections, and the evidence based Attachment and Biobehavioral Catch Up intervention, which helps caregivers respond sensitively to their infants and toddlers. Services are free, occur weekly for sixty minutes, and typically last six to nine months. The program serves caregivers in Hennepin and Scott counties and offers flexible weekday scheduling in the family's home.
