- HealthPartners Clinics patients having trouble paying their HealthPartners medical bills - Individuals must first apply for and use all government program assistance - If an individual was denied government assistance based on income guidelines, they may still qualify for the HealthPartners Clinics Financial Assistance Program

Interpretation services available
- Visit website for application/registration form. After the application is received, a letter is sent to the applicant within 10-14 days by mail notifying them if they qualify for the program or not. If they qualify, the amount the bill is discounted is included in the letter. The program qualification lasts three months from the approval date. Individuals need to apply again after three months to see if they qualify for the program. - Mail completed application to PO Box 183 Minneapolis, MN 55480-0183 - Email completed application to [email protected] - Walk in to clinic to submit an application. Put application and required documents in a sealed envelope labeled "Patient Accounting" to ensure confidentiality.

Offers a discount program for eligible patients and a payment plan option for any balance not covered by the discount. The program cannot be used for dental and pharmacy bills and charges. Program assistance is based on: - Assets, such as property or savings accounts - Family size - Income - Liabilities, such as bills or regular payments due Individuals may apply even if they have insurance coverage.
