Waiver Case Management - Keystone Community Services

Eligibility
  • Residents of Hennepin or Ramsey counties who are low-income, elderly, homebound, or persons of any age who live with a disability, and who are actively on the Community Access for Disability Inclusion (CADI) Waiver, Elderly Waiver or Alternative Care Grant programs.
  • Community Social Work Program serves families who have children in the White Bear Lake and North Saint Paul/Maplewood/Oakdale School districts.
