- If insured, please bring a copy of your health insurance, Medical Assistance card, or Medicare card. - If a family does not have insurance coverage, no documentation is required, and the same level of care will be provided. It is helpful if a family is able to bring copies of medical records with them to their appointment, but this is not required.
Eligibility
Children under the age of 18 with a treatable orthopedic condition are eligible to receive care. Income, financial status, and/or insurance coverage has no impact on eligibility for services.
