Customized Services - Living Well Disability Services

Provides staff support to individuals with disabilities who live on their own or are living with family members. Supportive services can be designed to help at home or at community settings. Services: - In Home Family Support (IHFS) - Independent Living Services (ILS) - Semi-Independent Living Services (SILS) - Support Living Services (SLS) - Respite temporary care Activities: - Community inclusion activities - Daily living and home care skills - Day services in the community (non-employment related) - Exploration and expansion of personal interests - Personal money management - Positive behavior support - Relationship building - Self-care skills Additional services developed or supported to meet individual needs.
