Description

Provides a social worker on staff to assist patients in a variety of ways. Helps identify the needs of each individual patient during a stressful time and suggests resources in the community for when they return home. Additional services the social worker can provide include: - Advance directives - Community Presentations - Consultation on child and vulnerable adult reports or suspicions - Crisis intervention - Discharge planning - Education on mental health, mandated reporting, and available resources - Mental health concerns - Referrals to community agencies - Support group facilitation The social worker can assist patients and their families whether they are inpatient, outpatient, through the Women's Health Center, or come in through the Emergency Department.
