Works with qualified individuals enrolled in the Hennepin County and Ramsey County CADI (Community Access for Disability Inclusion) Waiver, Brain Injury Waiver, and Elderly Waiver programs by connecting participants to an extensive range of home and community-based services, enabling people to live as independently as possible in community settings.
