Suspected fraud or abuse should be reported if the individuals are receiving benefits, health care providers working with the Minnesota Health Care Programs, or childcare providers receiving childcare assistance funds. Report fraud to the Minnesota Department of Human Services by following these suggestions: ?- Submitting the Program Integrity Oversight Intake Form (recommended). Although every report is reviewed, not all reports result in an investigation or action. ?- Depending on the nature of the report, individuals may be contacted for more information or follow-up. ?- Due to the volume of reports received, individuals should not expect to be contacted about their report.
