  • Adult caregivers or informal providers of in-home and community care for an older adult 60 years of age and older or an individual of any age with Alzheimer's disease or related dementias
  • Serves African-American and East African residents of Hennepin County

Provides supportive services for the African American and East African older adults and their caregivers through support groups, health monitoring and education, and respite care. Everyday Care: Provides caregiver support and coaching to older adults for healthy aging Alzheimer's Consulting: Dementia Consultants offer memory screening tools, Alzheimer's education classes, resources, and referrals Caregiver Support Groups: Provides culturally specific caregiver support groups for African Americans
