Provides home health and personal care services that enable older adults to continue to live independently. Home care services are provided in the community or in a Sholom-owned facility. Services include: - Home health aides - Nurse visits - Occupational therapy - Palliative care - Physical therapy - Remote patient monitoring to measure pulse oximetry, blood pressure, glucometers, temperature, and weight - Speech therapy - Social work services Home care services provided: - Alzheimer's and Dementia care - Bathing and incontinence care - Flexible care options, hourly, daily, and overnight care - Licensed, insured, bonded caregivers - Meal prep, housekeeping, errands, driving to appointments - Medication assistance - Transfers from bed to wheelchairs
