Provides home health care services in the metro area. Services offered: - Home Health Aides and Personal Care Aides: Help provide services that are essential to the individual's well being to prevent institutionalization. Services are provided intermittently, part-time, and up to 24 hours a day providing, bathing and grooming services, dressing, activities of daily living, transfers/walking, medication administration, meal preparation, and light housekeeping. - Homemakers: Provides basic homemaking tasks such as assisting the individual with errands, meal preparation, and companionship - Independent Living Skills Services (ILS)/Independent Community Living Supports (ICLS): Supports families in caring for family members who are able to remain in the home. Services are designed to honor each family's routines, values, and activities of daily living to foster independence in the home. Services include personal hygiene, dressing and clothing care, health care, cooking, eating, nutrition, home management, home safety, financial management, personal growth, problem solving, and community access. - Physical and Occupational Therapy: Offers licensed therapists to assist with pain management, cardiovascular rehabilitation, exercise to support strength, ambulation, and more - Skilled Nursing: Registered nurses can provide home visits and consult with doctors
