- Adults with developmental or cognitive disabilities who is a resident of Hennepin County, primarily in Minneapolis and first ring suburbs of Minneapolis - Adults who have had Special Education, an Individualized Education Plan (IEP), a psychological evaluation, or a referral from a case manager
Must have had Special Education or an Individualized Education Plan in high school, or other documentation of a cognitive disability
9 am - 5 pm Monday - Friday

Description

Provides highly individualized crisis intervention, advocacy, and navigation services that connects adults to community support systems that enable them to continue to live independently. Staff work directly with participants to identify and resolve barriers and typically accompany participants on initial meetings and appointments when facilitating connections. Services include but are not limited to:? - Access to medical support and follow-up on medical directives - Assistance in applying for publicly funded programs - Connecting to food support programs - Employment and volunteer readiness and placement - Home visiting and home management skills - Housing assistance - Ongoing one-on-one daily living skills counseling and general support - Safety planning - Shopping, budgeting, financial, and application assistance - Support with legal issues, daycare, school, and landlord/tenant issues - Waivered services
