Anyone receiving Minnesota services from an agency or licensed facility or program for developmental disabilities, mental illness, chemical dependency, or an emotional disturbance
Required documents
Call the Regional Ombudsman or central office to obtain a Consent for Release of Information form.

Receives and investigates complaints concerning an agency, licensed program or facility that provides services or treatment to individuals with mental illness, developmental disabilities, chemical dependency, or an emotional disturbance. May mediate or advocate on behalf of the client to resolve the complaint. Reviews the causes and circumstances of a serious injury or death of a client.
