Eligibility

Individuals ages 18 and older in good health, individuals ages 66 and older must provide letter from healthcare professional, weigh at least 110 pounds, and live within 40 miles of donation center

Required documents

  • Bring current ID with photo, signature, address, date of birth
accepted IDs include state ID, driver's license, passport, military ID, school ID
  • Bring Social Security number.
acceptable forms for Social Security number are Social Security card, W-2, or income tax report
  • Physical required for 1st time donation and annually thereafter
