Community Health Representative - Leech Lake Band of Ojibwe

Provides home health aid services such as: - Housecleaning - Personal grooming - Grocery shopping for elders - Food distribution delivery - Prescription delivery - Monitoring vital signs - Respite care services - Medical transportation to and from medical appointments - Out of town transportation with IHS referrals Please contact the CHR office at least two weeks in advance for transports. Priority list for services is as follows: - Elders - Diabetic/Dialysis patients - Heart/Cancer patients - Handicapped - General population CHR reserves the right to deny services to people with warrants, incarceration, intoxication, using illegal drugs, numerous cancellations, etc.

Primarily serves enrolled band members of a federally recognized tribe, with the exceptions of elderly non-natives or family members that live in Leech Lake Reservation and Beltrami, Cass, Hubbard and Itasca Counties.
