Pharmacy - Minnesota Community Care
Medication dispensing services are available for patients from any West Side Community Health Services' clinics. Accepts Medical Assistance, most Medicare Part D plans and most insurance. Uninsured patients receive sliding scale discounts based upon family income in addition to the lower priced drugs. For those still in need, pharmacists help the patient to apply for assistance through drug companies and explore other options to ensure patients receive the prescriptions they need. Medication Therapy Management (MTM): Goes beyond the standard counseling associated with receiving a new prescription. Pharmacists work directly with the doctor to assure that all of medication needs are being met. In private one-on-one MTM consultations, pharmacists assess the patient's entire drug therapy in order to answer any questions. The pharmacist works with the patient and the doctor to assure that all identified problems are resolved. Prescriptions for controlled substances cannot be filled.
