- Designed to help clients with serious and persistent mental health concerns secure needed community based mental health services - Case managers meet with clients on a regular basis, assess and prioritize their needs, arrange and coordinate services to be provided, monitor service delivery, secure entitlements and other benefits, and advocate for their client's rights - Case managers work closely with other community based providers to ensure that each client's individual needs are being met, and that successful community living is maintained
