Financial Aid Funds - Patient Advocate Foundation

Provides small grants to patients who meet financial and medical criteria on a first come first served basis and are distributed until funds are depleted. Open funds: - Heart Valve Financial Aid Fund: Supports patients who have been diagnosed with a qualifying heart valve condition or disorder and who are currently receiving treatment, have been in treatment within the last 6 months or are starting treatment within 60 days. Once approved, the financial fund provides a one-time $600 grant to help offset the out-of-pocket expenses associated with transportation, lodging, housing, utilities, and food needs related to heart valve treatments. - LEISHLine Financial Aid Fund: Supports patients who have been diagnosed and treated for Acute Respiratory Distress Syndrome (ARDS), Toxic Shock Syndrome (TSS), and/or Sepsis within the last 6 months. Once approved, the financial fund provides a one-time $1,000 grant to cover expenses related to transportation, housing and lodging, utilities, or food/nutrition needs. - Merkel Cell Carcinoma Financial Aid Fund: Supports patients who have been diagnosed with the rare form of skin cancer known as Merkel Cell Carcinoma (MCC). Once approved, the financial fund provides one-time assistance of $1,000 to cover transportation-related expenses connected to treatment appointments, lodging, and food or nutritional needs. Eligible patients include those who are undergoing active treatment, have received treatment within the previous 6 months, or will be receiving treatment in the next 60 days. - The Angel Fund: Supports funeral expenses associated with patients who have passed and were diagnosed and died from Acute Respiratory Distress Syndrome (ARDS), Toxic Shock Syndrome (TSS), and/or Sepsis within the last 6 months from the application date. Once approved, the financial fund provides a one-time $1,000 grant to cover funeral burial or cremation expenses.

Eligibility

Individuals residing in Minnesota must meet income guidelines and one of the following conditions: - Diagnosed with a qualifying heart valve condition and receiving treatment - Diagnosed and treated for Acute Respiratory Distress Syndrome (ARDS), Toxic Shock Syndrome (TSS), and/or Sepsis within the last six months - Diagnosed with Merkel Cell Carcinoma (MCC)
