Eligibility
In-home and community-based services available to youth who are in foster care and are residents of Anoka, Carver, Dakota, Douglas, Hennepin, Ramsey, Scott, Sherburne, Washington and Wright counties.
Youth receiving in-home counseling must be on Medical Assistance and be residents of Anoka, Carver, Dakota, Douglas, Hennepin, Ramsey, Scott, Sherburne, Washington and Wright counties in Minnesota.

Provides intensive, community-based mental health support for children and youth experiencing significant emotional or behavioral challenges. Offers individualized therapy, skill-building, crisis intervention, and family/foster support to improve daily functioning, stabilize symptoms, and reduce the risk of out-of-home placement.
Services are delivered in the home, foster placement, or community, with a focus on collaboration, flexibility, and empowering youth to thrive in their environment.
