- Home visits to provide antepartum, postpartum, breast feeding, family planning, infant care, child growth and development assessments, education and promotion of parenting - Provide services to children with special needs: skilled nursing visits, waiver case management, personal care attendant assessment and screening for Early Childhood Special Education services - Follow Along Program to provide information regarding growth and development and screen for need for referrals to other services including medical and Early Childhood Special Education. - Education related to injury prevention including home safety.
