Adults living with a serious and persistent mental health issue who are residents of Goodhue County, and meet one of the following criteria: - Commitment or stay of commitment as mentally ill in the past three years - Continuous psychiatric hospitalization or residential treatment of more than six years in the past year - Diagnosis of Schizophrenia, Bipolar Disorder, Major Depression, or Borderline Personality Disorder with significant impairment in functioning with a documented Mental Health Professional statement - Have had two or more episodes of inpatient psychiatric care in the past two years - Treatment by a crisis team two or more times in the past two years

As part of case management, supportive services may include: - Addressing medical and physical health - Advocacy - Financial management - Housing issues - Maintaining social relationships - Mental health management education - Mental health services - Transportation access - Vocational habilitation
