Eligibility
Adults living with a serious and persistent mental health issue who are residents of Goodhue County, and meet one of the following criteria:
- Commitment or stay of commitment as mentally ill in the past three years
- Continuous psychiatric hospitalization or residential treatment of more than six years in the past year
- Diagnosis of Schizophrenia, Bipolar Disorder, Major Depression, or Borderline Personality Disorder with significant impairment in functioning with a documented Mental Health Professional statement
- Have had two or more episodes of inpatient psychiatric care in the past two years
- Treatment by a crisis team two or more times in the past two years

Provides adult mental health targeted case management to assist individual's needs, develop person goals, and access needed services. As part of case management, supportive services may include:
- Addressing medical and physical health
- Advocacy
- Financial management
- Housing issues
- Maintaining social relationships
- Mental health management education
- Mental health services
- Transportation access
- Vocational habilitation
