Eligibility
  • African American persons living with HIV/AIDS who are at or below 300% of the federal poverty guidelines
  • Clients enrolled in the Medical Case Management program or Ryan White eligible for transportation, but do not receive medical transportation from another HIV provider
  • Primarily serves residents of Anoka, Carver, Chisago, Dakota, Hennepin, Isanti, Ramsey, Scott, Sherburne, Washington and Wright counties in Minnesota
