Provides transportation in the form of bus passes, and cab rides where utilizing public transportation is not an option, for core medical services including medical case management, HIV care and treatment, mental health, support groups, benefits counseling, and more.

Call for an appointment, cannot do same day requests; assessment completed which takes about 45 minutes

Application/Registration form; drivers license; health insurance card; Medical Assistance card; Medicare card; picture ID; proof of household income; referral form; state ID

Eligibility

- African American persons living with HIV/AIDS who are at or below 300% of the federal poverty guidelines - Clients enrolled in the Medical Case Management program or Ryan White eligible for transportation, but do not receive medical transportation from another HIV provider - Primarily serves residents of Anoka, Carver, Chisago, Dakota, Hennepin, Isanti, Ramsey, Scott, Sherburne, Washington and Wright counties in Minnesota
