Individuals must complete the following: - Apply for Medical Assistance; - Get an Assessment (Professional Statement of Need, Coordinated Entry Assessment, or MnCHOICES/Long-term Care Consultation) - Complete a person-centered plan. Individuals with a MA-funded case manager, request that Housing Stabilization Services be added to their service plan. Individuals without a MA-funded case manager, will need to find a housing consultant in their area to complete the plan. Individuals should bring the assessment to the person-centered plan meeting. - The MA-funded case manager or housing consultant will help individuals choose an ongoing provider for transition/sustaining services - The housing consultant or housing transition/sustaining provider sends the application materials to DHS for eligibility determination

The financial assistance is given to the approved agency to assist the client. The program provides a Medical Assistance benefit to help individuals with disabilities and seniors find and keep housing. Services consist of housing consultation services, housing sustaining services, and housing transition services.
